Published October 22, 2009 06:14 pm - H1N1 Vaccine info form Form requested to expedite vaccinations. Mankato health officials are asking parents bringing their children to the H1N1 vaccination clinic Saturday to fill out the following form. The free clinic is for children age 6 months to five years old. The vaccinations will be 10 a.m. to 3 p.m. Saturday at the ISJ Specialty Clinic, located at 1015 Marsh St. Mankato. Use entrance 2 on the north side of the medical center. A pre-recorded hotline with details is provided by calling 385-5600 Click here to access the H1N1 form.
